We are researchers and certified speech-language pathologists in the Department of Speech and Hearing Sciences at the University of Arizona. We are conducting a survey with ventilator users regarding speech and communication. It is our ultimate hope to use the feedback from these surveys to improve communication in people who use ventilators. The survey will provide us with insights into communication difficulties experienced by ventilator users, suggestions for how overall communication could be improved, and possible future research needs in this area. The results and recommendations from the survey should benefit ventilator users as well as those who live and work with such individuals.
We have been given permission to post this survey in hopes of hearing from some of you. If you are willing to participate, you may respond to this survey by responding directly to this email or by copying it and mailing it to us via regular mail. If you have any questions about the survey or your participation, please feel free to contact us by email (firstname.lastname@example.org) phone (520-621-2195), or mail at:

SUBJECT'S WAIVERYou are being invited to participate voluntarily in the above-titled research project. The purpose of this project is to gather information from ventilator users regarding the challenges of and potential improvements for ventilator-supported communication. You are being invited to participate because you use a ventilator to breathe. If you agree to participate, you will be asked to respond to questions from a survey form. We ask that you complete the survey in its entirety but you may stop at any point and we would still appreciate having the responses to the questions you did answer.
