BACKGROUND
Welcome to the RWSIR Post-Polio Program. This information packet is designed to help answer some of your questions before your first visit to the program and to make the experience more comfortable and beneficial. Polio survivors who are experiencing new health problems or who are concerned that they may have Post-Polio Syndrome (PPS) are understandingly apprehensive and frightened. Frequently there are questions about the causes of these problems, what can be done and what the future holds. The RWSIR Post-Polio Program is staffed by a team of health care professionals knowledgeable about PPS whose purpose is to solve new health problems, answer your questions, restore lost function and enhance your quality of life.

WHAT TO EXPECT FROM THE POST-POLIO PROGRAM
Information about post-polio syndrome is relatively new. Although we don't have all the answers, we have learned a great deal over the past several years. During your clinic visit we will try to identify those problems which seem to be related to your previous polio, and those which are not. As yet, there are no easy cures for PPS. There are, however, treatments for many associated problems, and treatments to minimize further progression, facilitate improved function and a sense of well being. For everyone, we can provide reassurance, education and the latest information. We strive to remain current with recent developments, new treatments and information, making referrals to other centers and resources as indicated.

CLINIC SCHEDULE
We have found that because of the nature and complexity of the health problems associated with PPS it is helpful to perform a multidisciplinary team evaluation. A complete evaluation consists of two days of outpatient services in Warm Springs. On Monday, you will check in to the out patient clinic where a nurse will first see you, and a fasting blood specimen will be drawn. A physician specializing in management of post-polio problems, a physical therapist, a certified orthotist, and frequently an occupational therapist will then examine you. Then you will have any diagnostic tests done that are felt to be indicated by the physician. These include pulmonary function tests, electrodiagnostic studies and radiological studies. On Tuesday, you will complete any evaluations, pick up your braces or seating clinic prescriptions and meet again with the physician to discuss the results of your diagnostic studies, to review your therapy recommendations and to pick up any medication or further therapy prescriptions.

SERVICES OFFERED

Physical therapy -- detailed manual muscle tests, gait evaluation, posture assessment, instructions in individual home exercise programs, therapy sessions for relief of symptoms in appropriate instances. Orthotics -- evaluation of current bracing, fabrication of individualized bracing and corsets and provision of manual and motorized assistive devices.

Occupational therapy -- evaluation of upper extremity function, evaluation of need for orthotics and home support systems to maintain or enhance daily living.

Medical -- evaluation of specific complaints and problems. Referral may be made for additional medical services or to other physician specialists, including neurologists or orthopedists.

Electrodiagnostic studies -- electromyography, nerve conduction studies (EMG/NCS).

Radiology. - X-Rays and Ultrasound, we will refer for MRI or CT.

THE POST-POLIO TEAM
Dr. Anne Gawne, MD, directs the team. Also included are Lorine Jackson, RN or Linda Durham RN, Pima McConnell PT, ATP, Linda Palmer, OTR .and Melvin Hale, CO.

FOLLOW UP
You may be seen for a follow up with Dr. Gawne in 2-4 months if any medications or therapies have been prescribed. Dr. Gawne in the Post-polio clinic follows many patients at regular intervals if needed. All patients are seen for yearly "follow up" visits. These appointments are less time consuming then the initial evaluation and generally include only pertinent laboratory or electrodiagnostic studies, and allow the patients time to get brace work or equipment repair done.

WHAT TO BRING:
You will need to bring the following items:
1. Any assistive devices you use or want evaluated (e.g. braces, crutches, corset and shoes).
2. Copies of any pertinent medical records, recent evaluations and X-rays.
3. A comfortable shirt and pair of shorts.

HOUSING
Rooms are available at Kress Hall and Builders Hall for a small charge. Health Insurance will not cover the cost of room and board under most policies. To make a reservation call (706) 655- 5176 . There is a cafeteria on the hospital grounds, and a few restaurants and fast food locations near by in Manchester.

PREADMISSION PACKET
In order to facilitate the registration process, please complete and return the enclosed questionnaire [please, contact the RWSIR Post-Polio Clinic for the packet and questionnaire] . The RWSIR post-polio questionnaire is intended to help us collect information about your current health status, your polio history. And your general medical history. Read over the entire questionnaire first, and then answer every question as carefully and completely as you can. Please return the questionnaire to

ADMITTING AND REGISTRATION
On the day of your first visit we will be getting some blood work in the morning. Be sure to eat nothing but do take all medications with a sip of water. Plan to arrive promptly at 8:30 for your initial visit on Monday. This will allow for time completing the preadmission process in the admission area in North wing I. Please sign in at the outpatient registration desk You will be asked to sign a Financial Responsibility Agreement form as well as a Consent and Release of Information form which gives your consent to release information to your insurance company or other physicians. It is helpful if you have their addresses and phone numbers when you come, so that they can receive copies of your records. If you like, you can receive a copy of your visit with an order from your physician.

FINANCIAL
All services are provided on a fee for service basis. RWSIR will file your insurance as a service for you. Private Insurance, Medicaid and Medicare cover most services. J E Hanger, who will file their clams separately, manages the Brace Shop. The cost of the two-day evaluation depends on the services provided.

CHANGING/CANCELLING OF APPOINTMENTS
Please make every effort to keep your appointment. If, however, you find it necessary to change your appointment, please notify Lorell Neely at (706) 655-5322 as soon as possible to arrange for an alternative appointment. We would appreciate at least 72 hours notice.
